Seagoing Service on Board GMU Ships
Procedure for undergoing Seagoing Service on the University's Training Ships Dar Młodzieży and Horyzont II :
- Students signing up for Seagoing Service for the first time are required to start a Seafarer's Service Book.
- The following documents must be provided to the Ship Management and Seagoing Service Department:
- A Maritime Authority application form completed in print form with the student's name according to their ID document. Where the student's residential address is different from the correspondence address, the residential address should be included on the back of the form. Students must submit a copy of their signature to the Department's staff.
- A colour photograph of size 35x45mm, taken in the last 6 months, on a uniform white background, reflecting the natural colour of the skin. The photograph must be sharp, and the eyes, especially the pupils, as well as the face from the top of the head to the top of the shoulders must be clearly visible. The face should take up 70-80% of the photograph.
The photograph should show the person in a forward-facing position without a head covering and glasses with dark lenses. The person should be looking straight ahead with fully open eyes, uncovered hair, a natural facial expression and a closed mouth.
Photographs that do not meet these requirements will not be accepted and delay the processing time of the Seafarer's Service Book.
- A Seafarer's Health Certificate printed in the required format, and confirming: no contraindications to work at heights above 1 metre; no typhoid or paratyphoid infection (Sanepid); and a negative result from a test for the presence of drugs and alcohol; or a separate certificate concerning the positive outcome of psychological examinations.
- A photocopy of the student's ID document. If the student refuses to provide a copy of his/her ID to the Seagoing Service Office, he/she must personally submit their documents and application for a Seafarer's Service Book to the maritime authority.
- Confirmation of payment (via bank transfer) of tax in the value of 100 PLN to the Maritime Authority.
